FOOD STAMPS The Food Stamp program is designed to raise the nutritional levels, extend food buying power and to safeguard the health and well-being among low income households. Depending on your household’s situation, you may be required to participate in various work activities to obtain or improve your employment. Each eligible family will receive the Ohio Direction Card to be used to purchase food at participating stores. Training to use the card is performed at our agency. MEDICAID Medicaid is a federal and state funded health program designed to provide medical coverage to low income individuals who are age 65 or older; blind, disabled; pregnant; children under age 21; and members of families who meet all eligibility criteria. The determination of health program eligibility requires the completion of an application and a scheduled face-to-face interview with a case manager. Healthy Start/Healthy Families is a free coverage for qualifying children and teens through age 19 and pregnant women who meet income guidelines. This program does not require an interview and resources are not considered. Minimal verification is required including income. An application can be obtained by calling 1-800-324-8680 or on the Internet at http://www.odjfs.state.oh.us/forms/pdf/07216.pdf OHIO WORKS FIRST (OWF) Formally ADC, provides time limited cash assistance to low income families with children under age 18 and low income pregnant women. Individuals receiving OWF are required to participate in work activities. An individualized self-sufficiency contract (SSC) is developed during a scheduled interview to define the participant’s obligations to receive benefits. Failure to fulfill the requirements of the SSC will result in loss of benefits to the family. ADULT PROTECTIVE SERVICES (APS) Services for adults 60 years of age or older at risk of abuse, neglect or exploitation. Investigation of a report will be initiated within 24 hours if an emergency exits; otherwise, the investigation will be made within 3 working days. All reporting information remains confidential. The caseworker will work with the adult, family and/or neighbors to arrange for services to assist the adult to remain safely in his/her home. CHILD CARE Financial assistance with child care expenses is available for children under the age of 13 while parents work or for those parents who are in training. Eligibility is based on the family size and household income using a percentage of the current Federal Poverty Level standard. Child care payments can only be made to Licensed or Certified Child Care providers. The agency is responsible for recruiting and certifying child day care providers. All eligibility for services is determined through an application process. An application can be mailed to you by calling (937)456-6205 or on the Internet at www.state.oh.us/odhs/cdc/form1138.pdf PREVENTION, RETENTION & CONTINGENCY (PRC) Local program to provide benefits and services to needy families to help overcome immediate barriers in achieving or maintaining self-sufficiency; prevent reliance on and diversion from ongoing cash; and overcome present crisis. To be eligible, there must be a child in the group or a pregnant woman. Income eligibility is base on a percentage of the current Federal Poverty Level. Resources are considered for eligibility. An application and interview is required. TRANSPORTATION Non Emergency Transportation (NET) program provides transportation for Medicaid recipients to non-emergent Medicaid providers. The consumer must be eligible for Medicaid to use NET transportation. Transportation is provided to advance scheduled destinations within the county and areas within a 35 mile radius. All transportation appointments are made by the agency with the contracted transportation provider. CALL TO REPORT FRAUD You do not have to identify yourself when making a fraud referral.
